La pr\u00e9sente politique de confidentialit\u00e9 d\u00e9crit les principes et m\u00e9thodes de la SCO visant la collecte de renseignements personnels, leur utilisation et leur communication. La SCO recueille, utilise et communique de tels renseignements seulement avec le consentement de la personne vis\u00e9e, \u00e0 moins que la loi ne permette ou n\u2019exige autrement. La collection, utilisation et communication de renseignements personnels par la SCO respectera la Loi canadienne anti-pourriel (LCAP) et sera r\u00e9gie par la version en vigueur de la pr\u00e9sente politique.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Renseignements personnels<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>Les renseignements personnels s\u2019entendent de toute information qui peut servir \u00e0 identifier une personne ou \u00e0 la joindre.<\/p>\n<p>La pr\u00e9sente politique s\u2019applique \u00e0 tous les renseignements personnels des membres et des membres \u00e9ventuels ainsi que des personnes ayant manifest\u00e9 de l\u2019int\u00e9r\u00eat envers la SCO ou ayant particip\u00e9 \u00e0 un \u00e9v\u00e9nement parrain\u00e9 ou organis\u00e9 par la SCO. D\u2019habitude, les renseignements personnels que la SCO recueille et utilise comprennent le nom de la personne, son adresse, son num\u00e9ro de t\u00e9l\u00e9phone, son adresse \u00e9lectronique, ses coordonn\u00e9es bancaires et sa carte de cr\u00e9dit (num\u00e9ro de la carte, type, date d\u2019expiration). La SCO demande aussi \u00e0 ses membres des renseignements pertinents pour leur adh\u00e9sion, tels que leur sursp\u00e9cialit\u00e9, leur certification, la facult\u00e9 de m\u00e9decine qu\u2019ils ont fr\u00e9quent\u00e9e, leurs principaux champs d\u2019int\u00e9r\u00eat et leur volont\u00e9 d\u2019assumer des fonctions au sein de la SCO.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Responsabilit\u00e9<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>La SCO a la responsabilit\u00e9 d\u2019assurer la confidentialit\u00e9 de tous les renseignements personnels en sa possession, y compris les renseignements qu\u2019elle achemine vers une tierce partie pour \u00eatre trait\u00e9s. La chef de la direction de la SCO a \u00e9t\u00e9 nomm\u00e9e agente de protection de la vie priv\u00e9e de la SCO. Elle exige d\u2019eux qu\u2019ils se servent de cette information uniquement aux fins pour lesquelles celles-ci leur a \u00e9t\u00e9 envoy\u00e9e et qu\u2019ils en assurent la confidentialit\u00e9 conform\u00e9ment \u00e0 la LCAP, aux lois pertinentes et \u00e0 la pr\u00e9sente politique.<\/p>\n<p>Les renseignements personnels recueillis par la SCO sont convers\u00e9s et trait\u00e9s \u00e0 Ottawa, Canada.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Buts de la collecte, de l\u2019utilisation et de la communication de renseignements personnels<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p><em><strong>Renseignements sur les membres<\/strong><\/em>\u00a0: La SCO recueille et utilise des renseignements sur ses membres et sur les candidats \u00e0 l\u2019adh\u00e9sion aux fins suivantes :<\/p>\n<ul>\n<li>traiter une demande d\u2019adh\u00e9sion ou de renouvellement d\u2019adh\u00e9sion<\/li>\n<li>recueillir et traiter les droits d\u2019adh\u00e9sion<\/li>\n<li>tenir les membres au courant des activit\u00e9s de la SCO et leur faire parvenir le\u00a0<em>Journal canadien d\u2019ophtalmologie<\/em><\/li>\n<li>informer les membres des activit\u00e9s qu\u2019organisent d\u2019autres organismes sur des sujets qui s\u2019inscrivent dans les vis\u00e9es de la SCO<\/li>\n<li>r\u00e9pondre aux questions et aux communications des membres<\/li>\n<li>informer les membres des r\u00e9unions, congr\u00e8s, ateliers et s\u00e9minaires qui sont organis\u00e9s par des organismes r\u00e9put\u00e9s et qui pourraient les int\u00e9resser<\/li>\n<li>analyser les donn\u00e9es d\u00e9mographiques des membres \u00e0 des fins de planification<\/li>\n<li>communiquer avec les membres pour sonder leurs opinions; l\u2019information recueillie contribuera \u00e0 orienter la SCO<\/li>\n<\/ul>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Communication de renseignements personnels<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>To streamline the membership application and renewal process for members of provincial associations or subspecialty societies affiliated with the COS, the COS collects membership dues on behalf of these organizations (for example the Canadian Retina Society and the Atlantic Provinces Ophthalmological Society). The COS then passes on the dues and the member\u2019s contact information to the other organization. Once information has been passed to the other organization, it is no longer in the control of COS and becomes subject to the privacy practices and procedures of the recipient organization.<\/p>\n<p>For the convenience of members, the COS prepares and makes available a directory of members. The online membership directory is available to members only and can be accessed in the members only section on the COS website. A member may request not to have their name included in the directory. Other than as outlined here, and as permitted or required by law, the COS does not disclose personal information unless it has obtained the individual\u2019s consent.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Consent<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>The COS uses personal information only with the consent of the individual, except as otherwise permitted or required by law. When an individual submits a membership application or membership renewal, or registers for an event, the individual will be given an opportunity to opt out of receiving further information from the COS or from any third parties. In addition, an individual may withdraw his or her consent to the use of personal information for any purpose at any time by contacting the COS at the address above. The COS will explain the consequences of withdrawing consent.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Website<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>The COS provides information and some member services via its website. By using the website, users agree that information may be collected, used and disclosed in accordance with this Policy.<\/p>\n<p><em><strong>Links:<\/strong><\/em>\u00a0The website contains links to other third party websites, such as those of COS Affiliates. These links are provided for the convenience of users. The COS has no responsibility or liability for, or control over, those websites or their collection, use and disclosure of an individual\u2019s personal information.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Accuracy<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>The COS seeks to ensure that the personal information it uses is accurate and up to date. Please assist the office by advising staff of any inaccuracies so that appropriate corrections can be made.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Limiting retention of personal information<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>The COS retains personal information as long as it is needed for the purpose for which it was collected and in order to meet legal and regulatory requirements.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Safeguards<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>The COS protects personal information against such risks as loss or theft, unauthorized access, disclosure, copying, modification and destruction by using appropriate security measures. Membership files are password-protected to ensure that personal information is kept confidential. COS employees with access to personal information are required to respect the confidentiality of that information pursuant to their employment agreement with COS. The COS implements methods of protecting personal information that include limiting access to the information, restricting access to offices where information is held and technological measures such as passwords.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Access<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>An individual may request access to the information held about him or her at any time by contacting the Privacy Officer at 613.729.6779 extension 225. The COS will endeavour to respond to any such request as quickly as possible and, in any event, within the legally required time periods. Subject to certain exceptions and limitations prescribed by law, an individual will be given access to any personal information held by the COS. The COS will correct or amend personal information that is shown to be incomplete or inaccurate.<\/p>\n\t\t  \t\t\t\t<\/div>\n\t\t  \t\t\t  <\/li>\n\t\t\t\t\t\t\t\t\t\t  <li class=\"accordion-item\" data-accordion-item>\n\t\t  \t\t\t\t<a href=\"#\" class=\"accordion-title\">Questions and complaints<\/a>\n\t\t  \t\t\t\t<div class=\"accordion-content\" data-tab-content>\n\t\t  \t\t\t\t  <p>For further information about this Policy or the practices of the COS related to personal information, or to make a complaint, please contact the COS Privacy Officer at 613.729.6779 extension 225.
